Ageing Health Summit

 

Integrated Care for Older Persons: From Assessment to Policy

The Ageing Health Summit is a full-day hybrid event (in-person and online) taking place during the TUFH 2026 Conference at the University of the Philippines, Manila. The summit focuses on implementing the WHO Integrated Care for Older Persons (ICOPE) framework through practical and scalable approaches, particularly suited for low- and middle-income settings.

Led by the WONCA Special Interest Group on Ageing and Health and The Network: TUFH’s Ageing Health Community of Practice, the summit brings together researchers, clinicians, community health workers, educators, students and policy leaders to explore integrated care models that support healthy ageing across diverse health systems.

 

The summit explores the full continuum of care for older persons, including:

      • Assessment and early identification of care needs

      • Care planning and service implementation

      • Monitoring and evaluation of outcomes

      • Task-sharing with community health workers

      • Interprofessional collaboration

      • Transitions of care across settings

      • Policy development at sub-national and national levels

The goal is to translate evidence into practical strategies that strengthen ageing health systems globally.

Program

  Keynote Address

This keynote address will introduce the World Health Organization’s Integrated Care for Older Persons (ICOPE) framework and explore its practical implementation across diverse health systems, particularly in low- and middle-income settings. The session will highlight strategies for early assessment of older persons’ needs, integrated care planning, and coordinated service delivery across community and clinical settings. It will also examine task-sharing approaches, interprofessional collaboration, and policy considerations that support sustainable, person-centred ageing care. Participants will gain insight into real-world implementation experiences, key challenges, and scalable solutions to strengthen integrated care for older populations globally.

 Multi-Stakeholder Panel

Panel

This multi-stakeholder panel brings together policy makers, clinicians, community health workers, and representatives of older persons to explore diverse perspectives on strengthening integrated care for ageing populations. The discussion will examine real-world challenges and opportunities in implementing person-centred ageing services across community and health system levels. Panelists will share insights on collaboration across sectors, the role of community-based care, and policy approaches that support sustainable and equitable ageing health systems, while ensuring that the voices and lived experiences of older persons remain central to care design and delivery.

  Interactive Workshop


This interactive workshop will engage participants in practical breakout sessions focused on implementing integrated care for older persons in diverse and resource-constrained settings. Participants will choose from three streams: training community health workers to conduct ageing health assessments, developing effective care plans with limited resources, and designing policy development frameworks to support integrated ageing services. Through collaborative discussion and knowledge sharing, participants will identify practical solutions and strategies, with key insights and recommendations synthesized into a summary document to inform future practice and policy.
